Understanding Skin Ageing: Intrinsic vs. Extrinsic Factors
When we observe fine lines, a loss of elasticity, and a deterioration in skin texture, it might seem like these are just unavoidable outcomes of growing older. Indeed, intrinsic ageing, which is the natural ageing process, plays a significant role in these changes. Our genetics largely determines this type of ageing and unfolds over time, irrespective of our lifestyle.
However, there is another type of ageing called extrinsic ageing, which we can influence through our choices and behaviours. External factors, including lifestyle habits such as smoking, excessive alcohol consumption, and an unhealthy diet primarily cause extrinsic ageing. Yet, the predominant factor in extrinsic ageing is exposure to sunlight and its ultraviolet (UV) rays, a process known as photoageing. This form of ageing speeds up the visible signs of getting older, and recent studies suggest that it accounts for most of the visible alterations to our skin.
Exposure to too much UV radiation is well-known as the primary cause of skin cancer in the UK. It also leads to other signs of skin damage. At Centre for Surgery, we offer several treatment options designed to reduce and reverse these changes. By understanding the differences between intrinsic and extrinsic ageing, individuals can take proactive steps to protect and rejuvenate their skin, maintaining a healthier and more youthful appearance for longer.
Skin Dehydration
Long-term sun damage can lead to dull, dry, and uneven skin in tone and texture. This occurs because sun exposure strips the skin of essential fatty acids and slows down cell renewal. In the outermost layers of the dermis, the water content decreases as the skin barrier gradually deteriorates.
To combat skin dehydration, one effective treatment option is the use of hyaluronic acid. Naturally present in the dermis, hyaluronic acid can hold up to 1,000 times its own weight in water, which helps the skin remain plump and hydrated. Profhilo, a treatment containing one of the highest concentrations of pure hyaluronic acid, deeply hydrates the skin. By improving moisture retention, Profhilo helps to create a smoother, more even complexion and skin tone.
Hyperpigmentation
Sun damage can lead to various changes in our skin, notably pigmentation concerns. Freckles, brown spots (also known as solar lentigos), and areas of hyperpigmentation or skin discolouration are all consequences of sun exposure. These changes occur because of the sun’s effect on melanocytes, the cells responsible for producing pigment and giving our skin its colour.
When exposed to the sun, our skin reacts by increasing melanin production. This natural defence mechanism helps protect the skin from sunburn and results in the tan that many people find desirable. However, when the melanocytes can’t keep up with the sun exposure, the skin burns, and repeated sun exposure can lead to an overproduction of melanin, known as hyperpigmentation.
Protecting your skin from further sun damage is crucial to managing and treating pigmentation concerns and avoiding the recurrence of hyperpigmentation. One effective treatment option is Fotona4D, which can significantly reduce or even eliminate pigmentary changes. For enhanced results, this treatment can be combined with the Obagi Nu-Derm system.
Crepey Skin, Collagen Loss and Wrinkles
Collagen and elastin are vital skin proteins that maintain firmness and suppleness. Starting in your mid-20s, the natural levels of these proteins begin to diminish, leading to the first signs of ageing. Sun exposure, however, can hasten this process and lead to crepey skin. This is why areas frequently exposed to the sun, such as the face, neck, décolletage, and hands, are more susceptible to wrinkles compared to skin that remains covered and protected.
When it comes to treatment, one effective option is Morpheus8. This advanced procedure stimulates the body’s natural production of fibroblasts, which are the cells responsible for generating collagen. By undergoing a course of Morpheus8 treatments, collagen levels can be significantly increased over 3-6 months.
Facial Redness, Thread Veins and Broken Capillaries
If you notice red, spidery veins on your cheeks and around your nose, it is likely a sign of sun damage. While genetics and conditions like Rosacea can also contribute to this issue, exposure to UV rays is a significant factor. UV exposure causes the skin to thin and the blood vessels to dilate, leading to the appearance of facial thread veins, telangiectasia, or broken capillaries.
One effective treatment for facial thread veins is the Nd YAG laser. This laser is specifically designed to target and treat facial blood vessels. It works by emitting a powerful light that is precisely tuned to the red pigment in the blood and blood vessels, leaving the surrounding skin unharmed. After an average of 3-4 sessions, patients typically experience up to 80% clearance of prominent blood vessels on the face.
Active Acne
Moderate sun exposure can sometimes have an anti-inflammatory effect, leading individuals with acne or blemish-prone skin to notice an improvement. However, excessive sun exposure can actually exacerbate acne problems. Even if your skin is oily, it still requires proper hydration. Too much sun dries out the skin, causing the sebaceous glands to produce more oil in an attempt to compensate. This overproduction of oil is a common contributor to acne. Additionally, excessive sun exposure slows down cellular turnover, hindering the shedding of dry skin cells and trapping oil and bacteria in the pores.
The same Nd YAG laser used for facial redness is highly effective for treating acne. This laser penetrates deeply into the skin, targeting and destroying the bacteria that cause acne while also reducing sebum production. Since acne often necessitates a multi-treatment approach, laser acne treatment can be combined with other therapies that promote cell turnover, such as gentle glycolic acid or fruit skin peels. These complementary treatments help to accelerate the shedding of dead skin cells, keeping pores clear and reducing the occurrence of acne.
