Single-stage hairline lowering surgery at Centre for Surgery costs from £7,500. Where greater advancement is needed and a two-stage procedure with a tissue expander is required, the total is from £11,500. It is performed as a day case under TIVA.
Hairline lowering (also called forehead reduction or scalp advancement) physically moves the hair-bearing scalp forward through an irregular trichophytic incision at the hairline, reducing forehead height by 1–5 cm in a single stage. Unlike a hair transplant, the result is visible immediately. It is an uncommon, technically demanding procedure that relatively few UK clinics offer. This guide explains what hairline lowering costs in London in 2026, what drives the price, how it compares with a hair transplant, and — importantly — who it is and isn’t the right procedure for.
How much does hairline lowering cost in London?
Pricing depends mainly on whether your scalp laxity allows a single-stage advancement or whether a two-stage approach with a tissue expander is needed, and on any procedures combined with it. The table below gives the indicative ranges; your exact quote is confirmed at your in-person consultation.
| Hairline lowering option | Indicative cost (London) |
|---|---|
| Single-stage hairline lowering | from £7,500 |
| With brow lift (combined) | from £10,000 |
| Two-stage with tissue expander | from £11,500 |
| With forehead contouring / as part of FFS | Individualised |
| Revision hairline lowering | from £2,000 extra |

The earlier “from £6,000” figures sometimes quoted around London understate what a properly resourced, consultant-led single-stage hairline lowering involves. Our pricing reflects a plastic surgeon, a CQC-regulated theatre, TIVA anaesthesia, and follow-up that runs to 12 months while the scar matures.
What drives the cost of hairline lowering?
- Single-stage vs two-stage: the single biggest factor. A two-stage procedure with a tissue expander involves two operations plus weekly expansion appointments, so it costs substantially more.
- Scalp laxity: how easily your scalp stretches determines how much advancement is possible in one stage. Limited laxity may push you toward the two-stage route.
- Combined procedures: adding a brow lift, forehead contouring, or hair transplant increases the total — though combining in one sitting is far more cost-effective than staging separately.
- Surgeon experience and facility: hairline lowering is uncommon and technically demanding; the quality of the trichophytic incision largely determines the final scar, so the operator matters more than a small price difference.
Single-stage vs two-stage: why the price difference?
Most patients have a single-stage procedure: a strip of forehead skin 1–5 cm wide is excised, the scalp is released and advanced forward, and the hairline is anchored in its new, lower position. One operation, one recovery.
Where more than about 5 cm of lowering is needed, or where scalp laxity is limited, a two-stage approach is required. A silicone tissue expander is placed under the scalp and gradually inflated with saline over 4–6 weeks to stretch the scalp, then a second operation removes the expander and advances the hairline. Two operations, a 4–6 week expansion phase with weekly clinic visits, and a visible bulge during expansion — all of which is why the two-stage route is from £11,500 rather than from £7,500. Your surgeon assesses scalp laxity at consultation and will tell you which route applies before you commit.
How much does hairline lowering cost combined with other procedures?
Hairline lowering is frequently combined with other upper-face procedures through the same incision or operating session:
- With a brow lift (when both brow descent and a high hairline need addressing): from £10,000
- With forehead contouring, or as part of a facial feminisation surgery (FFS) package: individualised pricing, typically a substantial saving versus staging separately
For trans women and non-binary patients, hairline lowering is a core component of FFS — reshaping a masculine M-shaped or straight hairline into a rounded, lower, feminine contour, usually alongside forehead contouring and a brow lift. If you are weighing the brow component separately, see our guide to how much a brow lift costs.
Hairline lowering vs hair transplant — which is better value for you?
This is the comparison that decides value for most people. Hairline lowering and an FUE hair transplant solve different problems. Hairline lowering moves the whole scalp forward, shortening the forehead immediately, with one scar hidden in the hairline — it is the right procedure for a stable, naturally high hairline with good scalp laxity. An FUE hair transplant adds individual follicles to build a denser or lower-looking hairline, grows in over 6–12 months, leaves no linear scar, and is priced by graft count — it is the right procedure for receding temples, thinning, or anyone with a hair-loss pattern. Spending from £7,500 on hairline lowering only makes sense if your concern is genuinely the position of a stable hairline; if it’s density or recession, a transplant is the better-value answer. Many patients ultimately benefit from both — lowering first, then a transplant to refine the temples or add density.
What’s included in the price of hairline lowering?
Your quote at Centre for Surgery is itemised and confirmed in writing. It covers:
- Your plastic surgeon’s fee
- Anaesthetist’s fee (TIVA)
- Theatre and facility fees at our CQC-regulated Baker Street hospital
- Tissue expander and fixation devices where used
- All pre-operative consultations and your pre-operative medical assessment
- Post-operative medications
- Suture removal at 7–10 days
- Surgeon reviews at 6 weeks, 3 months, 6 months and 12 months (the final scar review)
- Your surgeon’s direct number for the first 24 hours
- A dedicated patient coordinator
Hairline lowering needs extended follow-up because scar maturation continues for 6–12 months as hair grows through the trichophytic scar — that follow-up is built into the price, not charged separately.

Is hairline lowering worth the cost — and who shouldn’t have it?
For the right candidate, hairline lowering is good value: a single operation produces a permanent, immediately visible reduction in forehead height that no amount of styling can replicate. The patients who do best are women with a lifelong high hairline and no family history of progressive hair loss, and trans women and non-binary patients having it as part of FFS.
It is not worth the cost — and often not advisable at all — for some people, and we would rather say so plainly. The critical one is men with any family history of male-pattern baldness: lowering a hairline that then recedes genetically leaves a visible scar with no hair in front to camouflage it, so a hair transplant is usually the better and safer spend. Patients with very limited scalp laxity who don’t want a two-stage procedure, those with a previous coronal brow lift, and active smokers (smoking sharply raises the risk of a wide, visible scar) are also poor candidates. If your real concern is thinning or recession rather than the position of a stable hairline, the money is better spent on a transplant. We will tell you honestly at consultation which applies to you. Our articles on forehead reduction with a receding hairline and scarring after hairline lowering go into this in more detail.
Why choose Centre for Surgery for hairline lowering?
Hairline lowering is uncommonly performed in the UK — many clinics don’t offer it at all. Centre for Surgery’s team has specific experience with it, both as a standalone procedure and as part of facial feminisation.
- Specialist plastic surgeons: GMC-registered, members of BAPRAS and ISAPS, with specific hairline lowering experience for both congenital high hairline and FFS.
- CQC-regulated private hospital: our purpose-built hospital at 95-97 Baker Street is independently inspected and rated “Good” by the Care Quality Commission, with follow-up that runs to 12 months while the scar matures.
- All hairline-shortening options: single-stage, two-stage with tissue expander, FUE hair transplant, and combinations with brow lift or forehead contouring — so the recommendation is matched to your anatomy, not to one technique.
- Honest advice on suitability: hairline lowering is the wrong procedure for more enquirers than it is right for, and we say so — we don’t operate on men with significant family history of pattern baldness because the long-term scar outcome is likely to be poor.
- Trichophytic incision expertise: the final scar depends almost entirely on incision design and execution; our surgeons use carefully planned, beveled zig-zag incisions that let hair grow through the scar line.
- Two-week cooling-off period as standard, and finance through Chrysalis Finance.

Hairline lowering cost FAQs
How much does hairline lowering surgery cost in the UK in 2026?
At Centre for Surgery in London, single-stage hairline lowering costs from £7,500. A two-stage procedure with a tissue expander (for greater advancement or limited scalp laxity) is from £11,500 in total, covering both operations and the expansion-phase appointments. Combined with a brow lift, the total is from £10,000. Forehead contouring or an FFS package is priced individually. Your exact price is confirmed at consultation.
Why is two-stage hairline lowering so much more expensive?
A two-stage procedure involves two separate operations plus a 4–6 week expansion phase with weekly clinic visits to inflate a tissue expander under the scalp. That is far more surgical and clinic time than a single-stage advancement, which is why it costs from £11,500 rather than from £7,500. It is only used where more than about 5 cm of lowering is needed or where scalp laxity is too limited for a single-stage result.
Is hairline lowering cheaper than a hair transplant?
They are priced on different bases and solve different problems, so it isn’t a straight comparison. Hairline lowering is a fixed surgical fee (from £7,500 single-stage) that moves a stable hairline forward immediately. An FUE hair transplant is priced by graft count and adds density over 6–12 months. For a high but stable hairline, lowering is usually better value; for recession or thinning, a transplant is the appropriate spend. Many patients combine the two.
What’s included in the price of hairline lowering?
The price covers your surgeon’s fee, the anaesthetist (TIVA), theatre and facility fees, the tissue expander and fixation devices where used, all consultations and pre-operative assessment, suture removal, surgeon reviews at 6 weeks, 3 months, 6 months and 12 months, your surgeon’s direct number for the first 24 hours, and your patient coordinator. There are no hidden extras.
Is hairline lowering covered by the NHS or insurance?
No. Hairline lowering is considered a cosmetic procedure and is not covered by the NHS or private medical insurance. Rare cases as part of NHS-funded gender-affirmation care may have different funding pathways, but these are highly specific. It is otherwise funded privately with finance available.
How much does hairline lowering cost per month on finance?
Finance is available through Chrysalis Finance, subject to status; see our finance options page.
Is hairline lowering worth it for men?
Usually not, and we are conservative about offering it to men. Lowering a hairline that later recedes genetically leaves a visible scar with no hair in front to disguise it — so unless a man has very stable hair across multiple generations of family history, the money is better spent on a hair transplant, which adds density without depending on the hairline staying put. We discuss this candidly at consultation rather than simply taking the booking.
How much does hairline lowering cost as part of facial feminisation surgery?
Within an FFS package, hairline lowering is priced individually alongside the other procedures (commonly forehead contouring and a brow lift). Combining them in one operating session is substantially more cost-effective than staging them separately, and the comprehensive quote is provided at consultation after your surgical plan is agreed.
Does the price include revision if I need further advancement?
Revision hairline lowering, where further advancement is possible, is typically priced from £2,000 above the primary procedure. Most well-selected single-stage patients do not need revision; your surgeon will set realistic expectations about achievable advancement at consultation based on your scalp laxity.
