Breast lift with auto-augmentation — also called auto-augmentation mastopexy, auto augmentation mastopexy, mastopexy with auto-augmentation, breast auto-augmentation or breast lift with internal flap augmentation — is a specialised procedure that combines a breast lift (mastopexy) with the use of the patient’s own breast tissue to add volume to the upper pole, without implants.
Rather than using silicone implants to provide volume, auto-augmentation repositions the patient’s own lower pole breast tissue (a dermal or glandular flap) upward to fill the upper pole — producing a fuller, more projected upper breast from existing tissue. The procedure uses the same incision pattern as a standard mastopexy (lollipop or anchor), so no additional scarring is created.
This technique is particularly appropriate for patients who want to remove their existing breast implants but wish to maintain upper pole fullness without replacing them with new implants — and for patients wanting a modest volume enhancement with a lift using entirely their own tissue. Auto-augmentation provides improved upper pole fullness using only the patient’s own tissue — no silicone implants, no implant-related risks (capsular contracture, BIA-ALCL, implant rupture, need for future replacement).
The procedure redistributes existing breast tissue upward rather than adding external volume. This is particularly appropriate for patients who want their breasts to look fuller in the upper pole without a significant increase in overall breast size.

Breast lift with auto-augmentation is performed as a day-case at our Baker Street facility under TIVA (Total Intravenous Anaesthesia). The procedure takes 2–4 hours depending on complexity.
TIVA (Total Intravenous Anaesthesia) is administered by your consultant anaesthetist — an anaesthetic technique well suited to day-case surgery, using only intravenous agents with no inhaled gases.
The incision pattern follows the planned mastopexy technique — typically lollipop (vertical) or anchor (inverted T) depending on the degree of ptosis. For milder sagging that only needs a scar around the areola, a mini breast lift (doughnut mastopexy) may be more appropriate.
Excess skin is removed and the breast tissue reshaped. The nipple-areola complex is repositioned to a higher, more forward-facing position on the breast mound.
A flap of the patient’s own lower pole breast tissue and dermis is identified, carefully elevated, and repositioned upward to the upper pole of the breast — secured to the chest wall to maintain its new position. This redistributes existing tissue to provide upper pole fullness without implants.
All incisions are closed in layers using dissolvable sutures only — no removal appointment needed. A post-surgical bra is applied before discharge.
After the procedure, you recover in our monitored day suite before discharge with your responsible adult, post-operative medications, written care instructions, and your surgeon’s direct number for the first 24 hours.
Recovery after breast lift with auto-augmentation is similar to standalone breast lift recovery — the auto-augmentation component does not significantly extend the recovery period.
Rest at home. Wear post-surgical bra continuously. Avoid raising arms above shoulder height and any heavy lifting. Bruising, swelling, and temporary numbness of the breast skin are normal. Discomfort typically managed with paracetamol by the end of week 1. You may shower 48 hours after surgery. Wound check with our nursing team at 7–10 days — dissolvable sutures only, no removal needed.
Return to desk work typically possible by day 7–10. Light lower body activity from week 2. Continue avoiding upper body exertion. Surgical bra continues.
Surgeon review at 6 weeks. Full exercise including upper body resumes. Transition to underwired bra permitted. Apply silicone gel or tape to scars from 4 weeks to optimise scar quality.

Breast lift with auto-augmentation is appropriate for specific clinical situations. It is not a universally applicable alternative to implant augmentation — the volume increase achievable is modest and depends entirely on the amount of existing lower pole breast tissue available for repositioning.

A flap of lower breast tissue is moved upwards and secured to the chest wall while the breast is lifted. It uses the same lollipop or anchor incisions as a standard breast lift, so there are no extra scars.
Yes. Breast auto-augmentation is available in the UK, and at Centre for Surgery it is performed by GMC-registered plastic surgeons at our CQC-regulated Baker Street hospital. It is a day case under TIVA (total intravenous anaesthesia), so you go home the same day. Your surgeon first checks you have enough lower breast tissue for the flap, because the technique depends on it.
In a normal breast lift, the excess lower breast tissue is removed or simply reshaped. In auto-augmentation, that tissue is kept and moved up to fill the upper breast, giving more upper fullness and projection without an implant. The scars are the same. It suits moderate sagging with a modest volume goal, and needs enough lower breast tissue; if you have only mild sagging, a mini breast lift may be enough.
Neither is better for everyone, as they do different jobs. Auto-augmentation moves tissue already in the breast upwards as part of a lift, so it reshapes and lifts without a donor site. Fat transfer takes fat from the tummy, flanks or thighs by liposuction and injects it into the breast, adding new volume. If you want a noticeable increase in size, fat transfer to the breasts or implants give a larger, more reliable change.
Yes, and this is one of the main reasons for having it. After implants come out, the upper breast often looks deflated. Auto-augmentation can be done in the same operation as breast implant removal, lifting the breast and moving lower breast tissue up to restore fullness without new implants. That means one anaesthetic and one recovery. How much fullness it restores depends on how much tissue remains once the implants are out.
The cost of an auto-augmentation mastopexy is quoted after your consultation, once your surgeon has assessed how far the breast has dropped, the technique needed and the complexity of the flap. Yes. Auto-augmentation mastopexy is a significant operation, taking two to four hours depending on complexity, although you go home the same day. You are asleep throughout under TIVA, and a responsible adult must collect you and stay with you for 24 hours. Stop smoking at least four weeks before and after, because smoking significantly raises the risk of nipple tissue loss.
Recovery from an auto-augmentation breast lift is similar to a standard breast lift; the flap does not add much time. Most people return to desk work within 7 to 10 days and manage with paracetamol by the end of the first week. Wear your surgical bra continuously for six weeks after an auto-augmentation mastopexy. It supports the lifted breast and the repositioned tissue while they heal. You will see the final results of auto-augmentation at three to six months. At first the breasts are swollen and sit higher than their final position, then soften as the repositioned tissue settles. The downsides of auto-augmentation mastopexy are permanent lollipop or anchor scars, a modest gain in fullness and the usual risks of a breast lift. Part of the flap may also fail to settle, leaving some volume loss or an uneven contour. In a retrospective series of 151 women, Zaussinger and colleagues (2022) ↗ reported a 9% complication rate and no major complications. Nipple tissue loss is rare and more likely in smokers.
Over time, yes, to some degree. The repositioned tissue becomes part of the breast and the result is durable, but it ages like any breast tissue.
