FTM Top Surgery
FTM top surgery removes breast tissue for a flat, masculine chest, repositioning the nipples if needed. Choose it if you want a conventionally male chest; smaller chests may need only a scar around the areola.
Transgender and gender-affirming surgery in London for the chest, face, neck and body. Adults 18+ can self-refer with no GP letter or gender clinic referral, and the surgeon you meet at consultation is the one who operates.

Gender-affirming surgery here covers the chest, face, neck and body, with FTM top surgery, MTF top surgery, facial feminisation surgery and non-binary surgery each planned around the changes you want rather than a fixed package.
FTM top surgery removes breast tissue for a flat, masculine chest, repositioning the nipples if needed. Choose it if you want a conventionally male chest; smaller chests may need only a scar around the areola.
Non-binary surgery works to your goals, from a flat or smaller chest to changes to the face or body, without aiming at a male or female ideal. Choose it if neither conventional result fits you.
Body masculinisation surgery slims the hips and thighs and can add implants, for a straighter, more angular outline. Choose it if curves at the hips and thighs remain after top surgery.
MTF top surgery builds fuller breasts with implants, your own fat or both, once hormone-driven growth has levelled off. Choose it if hormones have given less breast growth than you hoped for.
Body feminisation surgery narrows the waist with liposuction and uses that fat to round out the hips and buttocks. Choose it if your waist and hips feel too straight.
Facial feminisation surgery changes the features people use to read a face as male, such as the brow ridge, jaw and Adam’s apple. Choose it if your face is what most often leads to misgendering.
A tracheal shave smooths a prominent Adam’s apple, leaving a short scar tucked into a crease high on the neck. Choose it alone if this is your main concern, or add it to FFS.
Not sure which of these fits you?
Answer a few short questionsA few short questions about the changes you want. You will get a suggested procedure and technique, realistic recovery times, and a clear answer on what we do and do not offer.
Gender-affirming surgery, historically called gender reassignment surgery, changes physical characteristics so that your body better reflects your gender. For many people it eases gender dysphoria; for others it is about feeling at home in their body. There is no required order and no single pathway. Some people have one procedure, others combine or stage several:
We do not offer genital gender-affirming surgery, such as feminising vaginoplasty, phalloplasty or metoidioplasty. These operations are carried out by specialist genital surgery units, usually after referral from a gender identity clinic.
No. Adults aged 18 and over can self-refer directly, with no GP letter or gender clinic referral. A psychological assessment letter is not a mandatory precondition for top surgery here, although we recommend one if you are in the earlier stages of gender-affirming care. Our guide to the WPATH letter for top surgery explains what one covers, and our comparison of NHS vs private FTM top surgery sets out routes and waiting times if you are weighing up the NHS pathway.
Not for every procedure, and the answer differs by operation. Testosterone does not usually need to be paused for top surgery, while oestrogen is reviewed case by case because of its effect on blood clotting. For MTF top surgery, implant size is usually planned once breast growth on hormones has settled. Our guides to hormone therapy before FFS and MTF breast augmentation without hormones cover the detail. Never stop prescribed hormones without advice from your surgical team and prescriber.
Top surgery, non-binary surgery and body masculinisation are performed by Dr Spyridon Vlachos and Dr Gino Vissers. Facial feminisation surgery and tracheal shave are performed by Mr Shehab Jabir, Consultant Facial Plastic Surgeon. The surgeon who assesses you at consultation is the surgeon who operates, under TIVA or local anaesthetic with sedation depending on the procedure, at our Baker Street hospital, rated Good overall by the Care Quality Commission.
You can see real patient results in our FTM top surgery before and after photos, and prices in our FTM top surgery cost guide. The clinical team uses inclusive language and welcomes all gender identities, and we recommend a two-week cooling-off period after every consultation.
