Brazilian Butt Lift — Before & After
33 photographs
A Brazilian butt lift — sometimes called a Brazilian bum lift, or simply a butt lift — is a fat transfer. Fat is removed by liposuction from one part of the body, processed, and injected into the buttocks to add volume and change their shape. Nothing is implanted and nothing is lifted in the surgical sense — the name is misleading. What limits the result is how much usable fat the patient has to give, and how much of the transferred fat survives in its new position. At Centre for Surgery the fat is placed under ultrasound guidance, which is discussed below.

360 Liposuction with BBL — Before & After
16 photographs
A BBL needs fat, and the fat has to come from somewhere. 360 liposuction takes it from around the full circumference of the trunk — abdomen, flanks and back — and that liposuction is doing as much work in the photographs below as the fat transfer is. Narrowing the waist changes the waist-to-hip ratio without a single millilitre being added to the buttock. The two effects arrive together and cannot be separated by eye.
Skinny BBL — Before & After
1 photograph
The skinny BBL is not a different operation. It is the same operation performed on a patient who has very little fat to give. Fat is harvested from wherever it can be found and the volume that can be transferred is correspondingly small, so the change is a matter of shape rather than size. Patients with limited fat reserves who expect a large-volume result are asking for something the procedure cannot deliver — and are sometimes better served by reconsidering whether they have enough fat for a BBL at all.
Hip Dips Fat Transfer — Before & After
1 photograph
Hip dips are the indentation where the hip bone meets the top of the thigh. They are largely a matter of skeletal shape rather than fat, which is why they do not respond to training or weight loss the way people expect. Hip dip surgery fills that depression with fat transferred from elsewhere on the body, softening the lateral line of the hip. The results in this section are hip fat transfer alone, so what changes is width and smoothness across the hip rather than the added buttock projection seen in the Brazilian butt lift sections above.
About These BBL Photos
The BBL photos above are presented for educational purposes only and should be interpreted subjectively. They do not represent the opinion or view of any patient regarding their result, and they are not intended to suggest the result that any other patient may achieve. No two patients are alike, and no two results are necessarily alike. Only a personal consultation with your surgeon can provide you with a reasonable expectation of the results you may achieve from any specific procedure. We only publish photographs of patients who have given full written consent for their images to be used.
How to Read BBL Before and After Photos
Everyone looks at the buttock. Look at the waist first. A BBL is a transfer, and the fat is taken from somewhere — usually the abdomen, flanks and back. Narrowing the waist enlarges the buttock in the photograph without a single millilitre being added to it.
Most of the shape was subtracted, not added
Two patients can receive the same volume of transferred fat and photograph completely differently, because one had 360 liposuction around the whole trunk and the other did not. Where an after photograph shows a dramatically narrowed waist and flat flanks, you are looking at the result of two procedures. That is a reasonable thing to do and an unreasonable thing to leave unstated. The combined cases above are labelled as such.
Not all of the transferred fat survives
Fat that is grafted has to establish a blood supply in its new position. A proportion of it does not, and is reabsorbed over the months after surgery. The proportion varies between patients and cannot be controlled precisely. This is the single most important thing a before and after pair cannot tell you, because an early photograph shows fat that has not yet been lost, alongside swelling that will also go. A photograph at eight weeks flatters. A photograph at a year is the truth.
How much fat the patient started with decided what was possible
There is no BBL without a donor site. A patient with generous abdominal fat has options that a lean patient does not, which is what the skinny BBL section is about. If the before photograph shows a body unlike yours, the after photograph is not a preview of yours.
Underwear, tan, posture, arch
Standing with the lower back arched projects the buttocks at no surgical cost whatever. A high-cut garment lifts; a tan deepens a shadow into a contour. In a trustworthy pair, the garment, the light and the stance are identical, and the only thing that has changed is the body.
What Stage of Healing You’re Looking At
A BBL photograph is unusually dependent on when it was taken, and in a direction that flatters the surgeon.
For the first weeks the buttocks are swollen and the donor sites are bruised, firm and often numb. A compression garment is worn. Patients are asked to avoid sitting directly on the buttocks for a period after surgery — typically two to three weeks, using a cushion to take weight through the thighs — because pressure on newly grafted fat compromises its blood supply.
Swelling settles over the first two to three months. Over the same period and beyond, some of the transferred fat is reabsorbed. These two things happen at once and pull in the same direction: the buttock gets smaller. Most surgeons regard the result as settled somewhere between six and twelve months.
Donor-site contour continues to refine over months, and skin retraction over the liposuctioned areas is slower still. A photograph at three months and a photograph at a year are both honest, and they are not the same photograph. A gallery that never states the interval is asking you to guess.
The after photographs in this gallery were taken at least six months after surgery, unless the caption says otherwise. For what to expect at each stage before then, see our BBL recovery guide.
What These Photographs Cannot Show You
There is a limit to what any gallery can tell you, and a clinic that pretends otherwise is selling rather than informing.
- How much of the fat survived. The result at six months is not the result at six weeks, and no photograph declares which one you are looking at.
- Whether 360 liposuction was performed. It changes the waist, and the waist changes how the buttock reads. See above.
- How much fat the patient had to begin with. The ceiling on any BBL is set by the donor site, not by the surgeon’s ambition.
- Donor-site irregularity. Rippling, dimpling and asymmetry across the liposuctioned areas are recognised outcomes and are, by a wide margin, the thing least likely to appear in a published gallery.
- Numbness and firmness. Altered sensation across the buttocks and the donor sites is normal for months and occasionally persists.
- What happens if weight changes. Transferred fat is living tissue and behaves like the rest of your fat. Gain weight and it enlarges; lose weight and the buttock shrinks with you.
- Whether a second procedure was needed. Some BBLs lose volume and are revised. Revisions are not usually photographed.
None of this is a reason to distrust before and after photographs. It is a reason to treat them as the beginning of a conversation with a surgeon rather than the end of one.
BBL Results: Common Questions
What does BBL stand for?
BBL stands for Brazilian butt lift. Despite the name, it is not a lift: fat is taken by liposuction from areas such as the abdomen, flanks and back, then transferred into the buttocks to add volume and shape. The liposuction also reshapes the waist, which is a large part of the change in the photographs above.
How long does a BBL last?
The fat that survives the first few months is permanent living tissue. A proportion is reabsorbed early on, which is why the buttocks look larger at first and settle over three to six months. After that, the result changes with your weight, like the rest of your body fat.
Will a BBL eventually sag?
The buttocks age like the rest of the body, so some softening and descent over the years is normal, and large weight changes speed it up. A BBL adds volume rather than lifting skin, so where there is significant loose skin, a buttock lift may be needed instead of, or as well as, fat transfer.
Brazilian Butt Lift (BBL Surgery) at Centre for Surgery
Centre for Surgery is a private hospital for plastic and cosmetic surgery on Baker Street in Marylebone, London, registered with and independently inspected by the Care Quality Commission. Our plastic surgeons perform Brazilian butt lift surgery at our Baker Street day surgery facility, including skinny BBL, male BBL, and BBL for hip dips, with or without 360 liposuction.
Ultrasound-Guided Fat Transfer
The principal safety consideration in BBL surgery is the plane into which fat is injected, and our guide to BBL risks explains how much difference that one decision makes to the death rate. Fat placed into or beneath the gluteal muscle can enter the large veins that run through it. Ultrasound guidance allows the surgeon to see the tissue planes during the injection and to keep the fat within the subcutaneous layer, above the muscle. This is the approach used at Centre for Surgery and it is consistent with current UK safety guidance on gluteal fat grafting. It does not eliminate risk, and no technique does. It addresses the mechanism by which the most serious complication of this operation occurs.
360 Liposuction Alongside BBL
Where a patient has fat available around the trunk, 360 liposuction both supplies the graft and reshapes the waist. Whether it is appropriate depends on where your fat sits and on the quality of your skin — liposuction removes fat and relies on the skin retracting over the smaller shape beneath, which is discussed in our liposuction gallery.
