Key Points
- How long is BBL recovery? Most people return to desk work after one to two weeks, drive at four weeks and go back to full exercise at six weeks. The final shape settles between three and six months.
- No sitting on your buttocks for two weeks. Sleep on your front or side, use a BBL cushion only if sitting is unavoidable, and sit normally for short spells from week three.
- The first five days are the hardest. Swelling and soreness peak on days three to five, then ease week by week.
- Wear your compression garment for six weeks, day and night. It covers the liposuction areas and leaves the buttocks free.
- Your bum will look smaller before it settles. Around 50 to 70% of the transferred fat survives, and the buttocks soften, or “fluff”, at around six to eight weeks.
- Know the warning signs. Breathlessness, chest pain or coughing up blood needs 999; a painful, swollen calf, a fever or a hot, red wound needs a call to your surgical team.
BBL recovery takes about six weeks before you are back to full activity. Most people return to desk work after one to two weeks, drive again at four weeks and go back to the gym, including weights, at six weeks. The result keeps developing after that: swelling peaks on days three to five, the buttocks fluff at around six to eight weeks, most of the result is visible at three months and the final shape settles between three and six months.
Brazilian butt lift recovery differs from most cosmetic surgery recovery as you are healing in two places at once. The liposuction donor areas, usually the abdomen, flanks and back, heal like any liposuction. The fat moved to the buttocks has no blood supply of its own yet, and how you sit, sleep and move in the first weeks decides how much of it survives. At Centre for Surgery a Brazilian butt lift (BBL) is a day case under TIVA (total intravenous anaesthesia), with fat placed only in the layer above the gluteal muscle using ultrasound-guided BBL technique. This guide sets out the BBL recovery timeline week by week, then sitting, sleeping, garments, massage, work, driving, flying, exercise, what to buy and the warning signs to act on.
How Long Is BBL Recovery?
BBL recovery time falls into four broad stages:
- Weeks 0 to 2, protection: rest, short walks, no sitting or lying on your buttocks, garment on day and night.
- Weeks 3 to 6, back to normal life: work and daily routines, short spells of sitting, light walking cardio.
- Weeks 6 to 8, full activity: garment off, full exercise including weights, and the buttocks begin to soften.
- Months 3 to 6, settling: the last swelling resolves and the surviving fat stabilises into your final shape.
So there are two answers: about six weeks to recover from the operation, and three to six months to see your result. Your pace depends on the volume of fat transferred, how much liposuction was done, your general health and how closely you follow your aftercare.

BBL Recovery Timeline: Week by Week
Day 1: the day of surgery
A BBL takes three to four hours, towards the longer end when combined with 360 liposuction. You are monitored for one to three hours and most people go home the same day, with a responsible adult who stays with you for the first 24 hours. Travel home lying across the back seat on your front or side if you can. The liposuction incisions are only 3 to 4 mm long and often leak blood-stained fluid for a day or two, so cover your bed and sofa with old towels. Get up and walk around the house that evening to help your circulation. For the first 24 hours you have your surgeon’s direct phone number.
Days 2 to 5: the worst days after a BBL
For most people, days two to five are the hardest. Swelling peaks around days three to five, bruising spreads and the liposuction areas feel sore and tight, like a deep muscle ache; the buttocks are usually less painful than the donor areas. Take your prescribed painkillers regularly rather than waiting for pain to build. Paracetamol is fine, but do not add anti-inflammatories, aspirin or supplements without checking with your team. Walk around the home every hour or two while awake, eat little and often and drink plenty of water.
Week 1 post-op BBL
By the end of the first week you will be moving more freely and needing less pain relief. The buttocks often look bigger than expected, which is mostly swelling. The rules stay the same: no sitting or lying on your buttocks, front or side sleeping, garment on day and night (off only to shower) and short walks. If you have a desk job and can avoid sitting, for example by working standing up, a return to work from around day seven is possible. The nursing team stays in touch over the first two weeks, and your nurse-led wound check takes place seven to 14 days after surgery.
Week 2 post-op BBL
At two weeks post-op, bruising is fading, most soreness has gone and you can manage longer walks outside. This is the last week of the strict no-sitting rule, and most people with office jobs are back at work by the end of it. The buttocks may feel firm or slightly lumpy, which is normal swelling in the grafted fat.
Weeks 3 to 6: back to normal life
From week three you can sit normally for short spells, getting up regularly. Light cardio in the form of brisk walking can start, but not cycling or spinning, which press on the buttocks. Many people feel like themselves again at four to five weeks post-op, although some swelling, numbness and tightness over the liposuction areas is normal. You can drive from four weeks. Keep wearing your garment until six weeks and avoid heavy lifting.
Weeks 6 to 8: the fluffing stage
At around six weeks you see your surgeon for a review, the garment usually comes off, and you can return to full exercise, building up gradually. Between six and eight weeks the buttocks start to soften and round out as swelling resolves and the skin and fat relax, a stage often called “fluffing”. Our guide to when fluffing happens after a BBL explains what it looks like. At eight weeks post-op most people are fully back to their normal routine.
3 months after a BBL
By three months most swelling has gone, most of your result is visible and most of the fat that will not survive has been reabsorbed. In a 2024 prospective study using 3D imaging, published in the Journal of Plastic, Reconstructive and Aesthetic Surgery ↗, 77.9% of the transferred fat volume was still present three months after ultrasound-assisted fat grafting above the muscle[1]. A review at three months is available.
6 months after a BBL and beyond
Between three and six months the final shape settles. In the same study, 64.7% of the transferred volume remained at six months[1], and a prospective ultrasound study of fat placed only above the muscle found the buttock fat layer thinned by an average of 18% between surgery and 12 months while keeping a clear overall gain[2]. From about six months the surviving fat is permanent and changes only with your weight. A review at six months is available.
Why Does My BBL Look Smaller? Will It Get Bigger After 6 Weeks?
It is normal for a BBL to look smaller at six to 12 weeks than in the first fortnight. Swelling is going down, and part of the transferred fat is being reabsorbed. Around 50 to 70% of the fat usually survives, which is why your surgeon slightly over-fills the buttocks at surgery. A 2026 review of 7,000 consecutive BBLs in Aesthetic Plastic Surgery ↗ estimated fat survival at around 70% with fat placed only under the skin[3].
Your BBL will not gain volume after six weeks, but it often looks better: as the tissue fluffs, the shape becomes rounder, and the contrast with your waist sharpens as liposuction swelling settles. If you are worried your result is shrinking more than expected, you can be seen sooner for an assessment, and a revision can be discussed at your six-month review. Deliberate weight gain to “feed” the fat is not needed; our article on how to feed fat after a BBL explains what actually helps.
When Can I Sit After a BBL?
Avoid sitting directly on your buttocks for the first two weeks. Pressure squeezes the tiny blood vessels growing into the new fat, and fat that loses its blood supply is reabsorbed or forms firm lumps. Uneven pressure on one side is also a common cause of asymmetry, as our guide on how to prevent dents after a BBL explains. If sitting is unavoidable, for example on a short car journey, use a BBL cushion that takes your weight on the backs of your thighs. From week three you can sit normally for short spells.
Using the toilet is fine throughout, including for bowel movements, but keep it brief. Our guide to when you can sit normally after a Brazilian butt lift covers cushions, car journeys and desk work in detail.
How Should I Sleep After a BBL?
Sleep on your front or side for the first two weeks, not on your back. A long body pillow, or pillows wedged behind your back, stops you rolling over, and a pillow under your hips makes front-lying more comfortable. After two weeks you can start sleeping on your back again. Our guide to how to sleep after a BBL includes pillow set-ups for front and side sleeping.
How Long Do You Wear a Compression Garment After a BBL?
Wear your compression garment, or faja, for six weeks, day and night, taking it off only to shower and wash it. It covers the liposuction areas and leaves the buttocks free, so it controls swelling and helps the skin settle onto your new waistline without squashing the transferred fat. Buy two so one can be washed. Garments are bought separately and confirmed before your operation. If yours digs in, leaves deep marks or causes tingling, tell your nursing team.
Is Lymphatic Massage Needed After a BBL?
Lymphatic drainage massage is optional. A 2026 systematic review in Plastic and Reconstructive Surgery Global Open ↗ found manual lymphatic drainage gave a moderate reduction in swelling after cosmetic surgery such as liposuction, with better results when started early (days two to five) and given over six to 12 sessions, though effects on pain and firmness were inconsistent[4]. If you have it, wait until your surgeon agrees, use a therapist experienced in post-surgical care, and avoid deep pressure on the buttocks. Our guide to lymphatic drainage massage after BBL surgery covers timing and sessions.
When Can I Go Back to Work After a BBL?
Plan for one to two weeks off. With a desk job, you may return from around day seven if you can avoid sitting on your buttocks, for example with a standing desk; most people with office jobs are back by two weeks. Jobs involving heavy lifting or physical work usually need longer, often up to six weeks, so discuss your role at consultation.
When Can I Drive or Fly After a BBL?
You can usually drive after four weeks, once you can sit comfortably, turn to check your blind spot and make an emergency stop, and are not taking painkillers that cause drowsiness. Check with your insurer too.
Long flights mean hours of sitting on the transferred fat, and immobility after surgery raises the risk of a blood clot in the leg (deep vein thrombosis). Avoid flying in the first weeks and agree the timing of any long-haul flight with your surgeon, who will usually advise waiting until you are back to full activity at around six weeks. Flying home within days is one of the main problems with a BBL abroad: it adds clot risk, clashes with the sitting rule and leaves complications to doctors who did not operate on you.
When Can I Exercise After a BBL?
Walking starts on day one and is the most useful exercise in the first fortnight. The usual progression:
- Weeks 1 to 2: short, frequent walks.
- Weeks 3 to 5: light cardio such as brisk walking; no cycling, spinning or heavy lifting.
- Week 6 onwards: full exercise, including weights, squats and running, after your surgeon’s review, starting lighter than your old routine.
Glute training will not burn off the transferred fat, and building muscle beneath it can add to the shape. Our guide to when you can work out after a BBL sets out a safe return to glute training.
What Do I Need for BBL Recovery?
A practical list of BBL recovery essentials to buy before surgery:
- Two compression garments, as advised by your team.
- A BBL cushion for unavoidable sitting, such as car journeys.
- A long body pillow to keep you on your front or side at night.
- Old towels or absorbent pads for the first days of fluid leakage.
- Loose, soft clothing that pulls on easily over the garment.
- A standing or raised work surface if you plan to work early.
- Your prescribed medication (bought separately), collected before surgery, plus paracetamol.
- High-protein, high-fibre food and plenty of water, stocked in advance to help healing and avoid constipation.
What Not to Do After a BBL
Good BBL aftercare is as much about what you avoid:
- Do not smoke or vape. Nicotine narrows the small blood vessels the fat depends on; stop all nicotine for four weeks before and four weeks after surgery.
- Keep alcohol to little or none for at least a week after surgery.
- Do not put ice packs or heat pads on the buttocks. Temperature extremes can damage healing fat, and numb skin burns easily.
- Avoid baths, hot tubs, saunas and swimming until your incisions have healed.
- Avoid tight clothing over the buttocks, such as skinny jeans, until your surgeon is happy.
- Do not lift heavy objects or train hard before six weeks.
- Do not crash diet or try to gain weight; a stable weight protects your result.
Is Recovery Different After a Mini BBL, Skinny BBL or 360 Lipo and BBL?
The two-week sitting rule applies to every type of BBL, but the rest varies:
- Mini BBL recovery time: around two hours of surgery with two donor areas and 800 to 1,000 ml of fat. Most people take about a week off, exercise from four weeks and drive at four weeks.
- Skinny BBL recovery: for people with a BMI below 23, harvesting modest volumes from four or five donor sites, so bruising is spread more widely, but the timeline matches a standard BBL.
- 360 liposuction and BBL recovery: treating the whole waist and back means more swelling and bruising around the torso early on and a longer operation, so the liposuction side takes a little longer to settle.
A tummy tuck is never combined with a BBL in the same operation at Centre for Surgery, as the two recoveries need opposite positions; they are staged three to six months apart.

What Are the Warning Signs After a BBL?
Most BBL recoveries are straightforward. In a 2024 study of 1,815 ultrasound-guided BBLs ↗, 4% of patients had a complication, most often a seroma (fluid collection), a small area of skin damage or a wound infection, with no fat embolisms or deaths[5]. A 2026 systematic review of more than 12,000 patients found a 2.67% rate of complications other than embolism, mainly contour irregularities, asymmetry, fat necrosis (firm lumps of dead fat) and infection[6].

Why Do the BBL Aftercare Rules Matter?
The most serious BBL risk arises during the operation itself. A 2017 task force report in the Aesthetic Surgery Journal ↗, covering nearly 200,000 gluteal fat grafting procedures, found that injecting fat into the deep muscle significantly increased the risk of fat embolism[7]. After surgeons adopted safer techniques, the reported death rate trended down from about 1 in 3,448 to about 1 in 14,952[8]. That is why fat is placed only above the muscle, with a blunt cannula, under ultrasound guidance on every BBL at Centre for Surgery. Fat in that layer relies entirely on the surrounding tissue for its new blood supply, so the sitting, sleeping, garment and no-smoking rules still matter in full.
Frequently Asked Questions
How long is BBL recovery time?
About six weeks to recover from the operation and three to six months to see your final result. Most people return to desk work after one to two weeks, drive at four weeks and resume full exercise at six weeks.
Can I go back to work 2 weeks after a BBL?
Yes, most people with desk jobs are back by two weeks, and some return from around day seven if they can avoid sitting on their buttocks, for example with a standing desk. Physical jobs involving lifting usually need longer, often up to six weeks.
What’s the worst day after a BBL?
For most people, days two to five are the hardest, with swelling peaking around days three to five and the liposuction areas feeling sore and tight. Regular prescribed pain relief, short walks and resting on your front or side help, and most people feel much better by the end of week one.
Can you sit to poop after a BBL?
Yes. Sitting on the toilet is fine throughout BBL recovery, but keep visits brief. Constipation is common after anaesthetic and painkillers, so drink plenty, eat fibre and ask your pharmacist about a gentle laxative rather than straining.
Will my BBL get bigger after 6 weeks?
Not in volume. Some transferred fat is reabsorbed over the first three months, so the buttocks usually look smaller at six weeks than at two. The shape often improves after six weeks, though, as swelling settles and the tissue fluffs into a rounder, more natural curve.
When does BBL fat stop dying?
Most of the fat that will not survive is reabsorbed in the first three months. In a 2024 3D imaging study, about 78% of the transferred volume remained at three months and about 65% at six months[1]. From around six months the surviving fat is permanent.
What should I expect at 8 weeks post-op after a BBL?
At eight weeks most people are back to normal, with the garment off, full exercise allowed and no sitting restrictions. Fluffing is usually under way, so the buttocks feel softer and look rounder, and the result keeps refining until three to six months.
Can a BBL go flat after 2 years?
Fat that has survived six months is permanent, so a BBL does not go flat on its own. It can look smaller after significant weight loss, as the transferred fat shrinks like your other body fat. If volume loss was greater than expected, further fat grafting may be possible.
