Key Points
- Loose, sagging skin on the thighs is very common after major weight loss, most often on the inner thighs, and it rarely tightens enough on its own.
- Exercise and non-surgical skin tightening help mild laxity. A thigh lift is the only way to remove a significant amount of loose thigh skin.
- The type of thigh lift depends on where the loose skin is: a groin-crease (horizontal) lift for the upper inner thigh, a vertical lift for skin running down to the knee, or a lower body lift for the outer thighs.
- If the main problem is fat rather than skin, liposuction may be enough; many people need both.
- Every thigh lift leaves a permanent scar, and wound healing problems are the commonest complication.
- Your weight should be stable for at least six months, and ideally 12 months after major weight loss, before surgery.
After losing a lot of weight, many people find their thighs are left with loose, sagging skin that rubs when they walk, makes clothes uncomfortable and does not respond to exercise. This guide explains why loose skin on the thighs happens, what you can do about it without surgery, how a thigh lift works, and how to tell which type you may need.
At Centre for Surgery, thigh lifts are performed by GMC-registered plastic surgeons at our Baker Street hospital in London, which is CQC-regulated and rated Good overall. For loose skin on other parts of the body, see our guide to loose skin after weight loss.
Why Do Thighs Sag After Weight Loss?
Skin that has been stretched over a larger body for a long time loses much of its ability to spring back, as the collagen and elastin fibres that give it firmness are damaged. When the fat underneath is lost, the skin envelope is too big for the thigh, and it hangs or folds. The inner thighs are particularly affected, as the skin there is thin and naturally less firm. Age, genetics, sun damage, smoking and how quickly the weight came off all make a difference.
Surplus skin is close to universal after massive weight loss: in a survey of 252 people after gastric bypass in Obesity Surgery ↗, 96% developed surplus skin, and the thighs were among the areas people most wanted treated.[1]
How to Tighten Loose Skin on Thighs After Weight Loss
For mild laxity, these steps can help, and they are worth doing whatever you decide later:
- Strength training: squats, lunges and inner-thigh exercises build muscle that fills out the skin and improves tone.
- Keep your weight stable and eat enough protein, particularly after bariatric surgery.
- Do not smoke, and protect your skin from the sun.
- Be realistic about creams. Moisturisers improve how skin feels, but no cream tightens a significant fold of loose skin.
Exercise cannot shrink skin that has been stretched significantly. If you can gather a fold of skin in your hand along the inner thigh, exercise alone is unlikely to remove it.
Non-Surgical Options for Saggy Thighs
For mild loose skin, energy-based treatments can tighten the skin over several months:
- Morpheus8 combines microneedling with radiofrequency to improve firmness and texture, with little downtime.
- BodyTite delivers radiofrequency beneath the skin, usually with liposuction, for a stronger tightening effect on the inner thighs or knees.
These tighten what is there but do not remove skin, so they suit mild laxity rather than hanging folds.
Thigh Lift, Thigh Liposuction, or Both?
| Your main concern | Likely option |
|---|---|
| Stubborn fat, with good skin tone | Thigh liposuction |
| Loose, sagging skin with little fat | Thigh lift |
| Both fat and loose skin | Thigh lift with liposuction |
Liposuction on its own can make loose skin look worse if skin quality is poor. Adding liposuction to a thigh lift, where there is fat to remove, is common. A 2025 systematic review of 1,113 patients ↗ found that liposuction-assisted medial thigh lifts had fewer complications overall than lifts without it (36.8% vs 70.7%), including fewer seromas and infections.[3] If you are unsure whether you have loose skin or fat, our guide to loose skin vs fat explains the pinch test, and our guide to losing thigh fat covers the non-surgical side.
Types of Thigh Lift
Inner (Horizontal) Thigh Lift
Removes loose skin from the upper inner thigh through a scar in the groin crease, lifting the skin upwards. It suits mild to moderate laxity close to the groin. A small version is often called a mini thigh lift.
Vertical (Medial) Thigh Lift
Adds a scar down the inner thigh and removes skin around the width of the leg, often combined with a groin scar in a T shape. It suits loose skin running down the inner thigh towards the knee, which is typical after major weight loss. Our guide to horizontal vs vertical thigh lift compares the two in detail.
Outer Thigh Lift
Loose skin on the outer thighs is lifted from above, through a scar around the hips, usually as part of a lower body lift or buttock lift.
Am I Suitable for a Thigh Lift?
You may be suitable if you:
- Are aged 18 or over and in good general health.
- Have loose skin on the thighs that has not improved with exercise.
- Have had a stable weight for at least six months, and ideally 12 months after major weight loss.
- Do not smoke, or will stop for at least four weeks before and four weeks after surgery.
- Accept the scar, and have realistic expectations.
A higher BMI increases the risk of complications, so your surgeon may advise losing more weight first.
How Much Skin Can Be Removed in a Thigh Lift?
It depends on your skin quality, how much loose skin there is and your anatomy. The aim is a firmer thigh without excessive tension. Pulling the skin too tight can affect blood supply and lymphatic drainage, cause the scar to spread or drift, or pull on the labia in women. Your surgeon will plan the amount removed with this balance in mind, and anchors the deeper tissue to strong structures in the groin to support the lift.
Can a Thigh Lift Create a Thigh Gap?
Removing loose skin, sometimes with liposuction, can create more space between the thighs. But a thigh gap depends largely on your bone structure and hip width, which surgery does not change. Most people having a thigh lift after weight loss are aiming for comfort, less chafing and a better fit in clothes, rather than a specific look.
Can a Thigh Lift Help Saggy Knees?
A thigh lift mainly treats the upper and inner thigh. Its effect on loose skin around the knees is limited. A vertical lift can be extended towards the knee in some cases, and liposuction or BodyTite can help mild fullness or laxity above the knee. Your surgeon can advise on the options at consultation.
What Scarring Will I Have After a Thigh Lift?
Every thigh lift removes skin, so every thigh lift leaves a permanent scar. An inner thigh lift scar sits in the groin crease, where underwear and most swimwear hide it. A vertical lift adds a scar along the inner thigh, visible when the legs are apart. Scars are pink and raised at first, then fade and flatten over 12 to 18 months. Our guide to thigh lift scars covers scar care.
The Thigh Lift Procedure
A thigh lift at Centre for Surgery is performed under TIVA (total intravenous anaesthesia), delivered entirely through a vein rather than via inhaled gases. The skin is marked with you standing. Liposuction is carried out first if needed, then the loose skin is removed, the deeper tissue is anchored and the wound is closed in layers. Dressings and a compression garment are applied, and most people go home the same day.
Thigh Lift Recovery
- Swelling, bruising and tightness are expected; prescribed pain relief keeps you comfortable.
- Walk gently from the first day to help circulation and lower the risk of blood clots.
- Avoid spreading your legs wide or sitting with legs apart while the wounds heal.
- A compression garment is usually worn for around six weeks.
- Most people return to desk-based work after one to three weeks, depending on the type of lift.
- Strenuous exercise is avoided for at least six weeks.
Our thigh lift recovery guide covers aftercare in detail.
Risks of Thigh Lift Surgery
Thigh lifts have a relatively high rate of minor complications, mainly related to wound healing. In a 16-year study of 151 thigh lifts ↗, wound healing problems were common, but only 12% needed surgery to treat them. Older age was linked to wound problems, smoking to scar problems, and a higher BMI to more complications overall.[2] Other risks include seroma, infection, temporary or lasting leg swelling, scar widening and changes in sensation. Our guide to reducing complications from skin removal surgery explains what you can do.
How Much Does a Thigh Lift Cost?
The cost depends on the type of thigh lift and whether liposuction is included, and is confirmed after your consultation. Current prices are in our thigh lift cost guide. Finance is available, subject to status. Our guide to skin removal surgery on the NHS explains when NHS funding may apply.
Frequently Asked Questions
Will loose skin on my thighs tighten on its own?
Mild laxity can improve over the first year after weight loss, especially with strength training. Significant loose skin after major weight loss usually does not tighten enough on its own.
How do I get rid of loose skin on my inner thighs?
For mild laxity, exercise and radiofrequency treatments can help. For a fold of loose skin, an inner thigh lift is the only way to remove it.
Is a thigh lift better than liposuction?
They treat different problems. Liposuction removes fat; a thigh lift removes skin. Many people after weight loss benefit from both.
Where are the scars after a thigh lift?
In the groin crease for an inner thigh lift, plus along the inner thigh for a vertical lift. Outer thigh lifts leave a scar around the hips.
How long after weight loss can I have a thigh lift?
Once your weight has been stable for at least six months, and ideally 12 months after major weight loss.
Does a thigh lift stop chafing?
It often reduces rubbing and chafing between the thighs by removing the loose skin that causes it.
Can a thigh lift be combined with other procedures?
Yes, it is sometimes combined with liposuction or other body contouring. Extensive work is often staged to keep each operation safer.
