Key Points
- A BBL in Turkey can be done safely, but it is harder to check that it will be. Safety depends on the surgeon, technique, hospital and aftercare, and most of these are difficult to verify from the UK.
- Turkey accounts for most UK complications after cosmetic surgery abroad. In the BAAPS national database, 76% of 198 patients treated in the UK had their surgery in Turkey, and 48% needed an operation once home.
- BBL deaths are caused by fat entering the gluteal muscle. Fat injected into the muscle can reach large veins and travel to the lungs; fat kept above the muscle under real-time ultrasound avoids that mechanism.
- Flying home soon after a BBL adds risk. A flight raises the risk of blood clots and clashes with the two weeks when you should not sit on your buttocks.
- Aftercare is the weak point of BBL abroad. Packages usually end at the airport, and complications at home are treated by doctors who did not operate.
- The lower price leaves out the cost of things going wrong. Revision and readmission are usually excluded, and the NHS treats emergencies but does not fix cosmetic results.
A BBL in Turkey can be safe when an experienced plastic surgeon places fat only in the layer above the gluteal muscle under real-time ultrasound, in a properly equipped hospital, with aftercare that continues after you leave. The difficulty is that a lower-cost package makes most of those things hard to check from home, and UK data show that most patients who return with complications after cosmetic surgery abroad had their operation in Turkey[1]. The Foreign Office reports that it is aware of 7 British nationals who died in Turkey in 2025 following medical procedures[2].
This guide sets out what the evidence says about BBL in Turkey and BBL abroad, without scaremongering: why a Brazilian butt lift carries more risk than most cosmetic surgery, why that risk grows when you travel, how to check a surgeon abroad, and what to do if something goes wrong at home. Our guide to BBL risks covers every complication, and our BBL death rate and safety statistics page brings the global data together.
Is BBL in Turkey Safe?
There are skilled plastic surgeons in good hospitals in Turkey, and there are high-volume clinics competing mainly on price. The country itself does not make a BBL safe or unsafe. What matters is one technical question, then a set of practical ones that get harder abroad.
The technical question is where the fat goes. Gluteal fat grafting has carried a higher death rate than any other aesthetic operation. A 2017 survey of 692 surgeons by the ASERF Gluteal Fat Grafting Task Force, published in Aesthetic Surgery Journal ↗, captured 198,857 BBLs with 32 deaths from pulmonary fat embolism, and found that surgeons who injected into deep muscle had significantly higher rates of fat embolism[3]. A follow-up survey found the estimated death rate fell from 1 in 3,448 to 1 in 14,952, while the share of surgeons injecting into deep muscle fell from 13.1% to 0.8%[4].
The British Association of Aesthetic Plastic Surgeons (BAAPS) advised its members not to perform BBL in 2018. In 2022 it set out the conditions for gluteal fat grafting: fat injected into the subcutaneous plane only, with intraoperative ultrasound used during fat placement to keep the cannula in that plane[5].
Real-time ultrasound makes the safe plane checkable. In a 2023 series of 4,150 ultrasound-guided BBLs, ultrasound confirmed that fat consistently stayed above the deep gluteal fascia, in the subcutaneous space[6]. Our guide to ultrasound-guided BBL explains how this works in theatre.
So a safe BBL in Turkey is possible. The hard part is confirming the plane and ultrasound use before you pay, knowing that the surgeon you researched is the one who operates, and knowing who looks after you once you are back in the UK.
Why Is BBL Cheaper in Turkey?
Advertised BBL packages in Turkey are often less than half the price of a UK operation, and some of that saving is genuine. Staff, property and running costs are lower than in central London, high-volume clinics spread costs across many patients a day, and medical tourism is actively promoted. Many packages bundle a hotel, transfers and a translator.
The rest of the difference usually comes from what the package leaves out:
- Who operates. In some packages the surgeon you chose is not the person who performs your BBL.
- Ultrasound guidance. Real-time ultrasound adds equipment and theatre time.
- Clinical observation. A hotel bed is not a monitored recovery.
- Follow-up. Aftercare often ends at the airport.
- Complications and revision. Readmission and corrective surgery are usually excluded, or limited to a short window at your own travel cost.
Cost is the main reason people travel. In a 2026 study of patients treated for complications at a London NHS plastic surgery unit, 40 of the 48 surveyed said cost was their main motivation, 26 regretted having surgery abroad and 32 said they would not make the same decision again[7]. Our guide to how much a BBL costs in London explains what a UK fee includes.
What Does UK Data Show About Complications After Surgery Abroad?
The clearest UK picture comes from the BAAPS Cosmetic Tourism Complications Database. Its 2026 national analysis, published in Aesthetic Surgery Journal ↗, covered all 198 cases reported between September 2022 and September 2024[1]:
- 76% had their surgery in Turkey. Most patients were women (93%), with an average age of 39.
- Tummy tucks were the most common procedure (45%), followed by breast surgery and liposuction.
- Wound breakdown (37%), infection (28%), seroma (24%) and tissue death (20%) were the most frequent problems.
- 48% needed an operation in theatre back in the UK and 4% needed intensive care. One patient died from a pulmonary embolism.
- Inadequate aftercare (21%), poor preparation (12%) and flying too soon (5%) were recurring contributing factors.
The researchers estimated the NHS cost at between 1.2 and 1.8 million pounds over two years. These figures cover all cosmetic surgery, not BBL alone, but earlier NHS series show that buttock surgery features heavily and can be severe. At one London major trauma centre, gluteal augmentation was the second most common procedure among returning patients, and all four major complications followed gluteal augmentation[8]. At a London teaching hospital, gluteal enhancement was the most common procedure among patients needing surgery for complications (38%), infection was the main problem (92%) and the average stay was 8 days[9].
A 2026 rapid review in BMJ Open gathered 35 NHS case series and reports describing 655 patients treated after elective surgery abroad, 265 of them after cosmetic surgery and most of the rest after weight-loss surgery. Turkey was the most common destination (61%), and the authors noted that nobody systematically records how many people travel or return with problems[10], so the published numbers are likely to be an undercount.
BBL Turkey Deaths: What Is Known?
Deaths after BBL in Turkey are rare in absolute terms, but they happen and follow a recognisable pattern. The Foreign, Commonwealth and Development Office (FCDO) states that it is aware of 7 British nationals having died in Turkey in 2025 following medical procedures, and that others have needed further treatment or surgery[2]. That figure covers all medical procedures, including weight-loss surgery, not BBL alone.
The clearest UK account of a BBL death in Turkey comes from a coroner. A 2023 Prevention of Future Deaths report ↗, written after a young British woman died following a BBL in Turkey, heard expert evidence that fat had been injected into the superficial muscle, recorded that she was not seen by a surgeon before the day of the procedure, and raised concern that patients travelling to Turkey for BBL are not being made aware of the risks[11].
The same pattern appears elsewhere. A 2023 autopsy study of BBL deaths in South Florida, published in Aesthetic Surgery Journal ↗, found that in every fat embolism death fat had been injected into the gluteal muscle at several levels, 92% of patients were operated on at high-volume, budget clinics, and operations took around 90 minutes. Fourteen of the region’s 25 deaths between 2010 and 2022 happened after safety guidelines and a subcutaneous-only rule were in place[12]. Rules alone do not make a BBL safe: time pressure, high throughput and budget pricing can undo them in any country.
Why Is a BBL Abroad Riskier Than Other Surgery Abroad?
Every operation abroad shares the problems of travel and fragmented aftercare. A BBL adds risks of its own.
The Plane of Injection and Ultrasound
The gluteal muscle contains large, thin-walled veins. If a cannula enters or passes beneath the muscle, fat can be drawn into those veins and carried to the heart and lungs within minutes. Without imaging, the surgeon judges depth by feel, and a cannula can slip between layers without any obvious change. From the UK, you cannot see whether a clinic uses ultrasound throughout fat transfer or simply owns a scanner.
Fat Volumes and Operating Time
Large volumes injected quickly raise tissue pressure and leave less margin for error. At Centre for Surgery, a full BBL typically transfers around 1,600 to 2,000 ml and takes 3 to 4 hours, while a mini BBL uses 800 to 1,000 ml over about 2 hours. A package promising a very large volume in a short slot is a warning sign, as is extensive 360 liposuction plus a tummy tuck and BBL in one session. At Centre for Surgery a tummy tuck and BBL are always staged 3 to 6 months apart.
Flying Home and Blood Clots
Major surgery and air travel each raise the risk of a blood clot in the leg, which can travel to the lungs. The NHS advice on cosmetic surgery abroad ↗ says to avoid flying for 5 to 7 days after liposuction and longer after a tummy tuck, and warns against holidays straight after surgery[13]. A BBL combines liposuction of several areas with fat grafting, and the flight home from Turkey usually lasts around four hours. In the BAAPS database, flying too soon was a recurring factor in complications[1]. Our guide to how soon you can travel after cosmetic surgery covers clot prevention on long journeys.
The Sitting Rule
After a BBL you should not sit directly on your buttocks for 2 weeks, as pressure reduces blood supply to the grafted fat and affects how much survives. The taxi, the departure lounge and the flight all involve sitting, and a BBL cushion does not replace lying on your front or side. Our guide on when you can sit after a BBL sets out the timeline.
Aftercare and Follow-Up
Fat embolism can happen in theatre or within hours, while infection, seroma, fat necrosis and wound breakdown usually appear over the following days and weeks, often after you have flown home. Swelling peaks at 3 to 5 days, the skin softens or “fluffs” at around 6 to 8 weeks, and most of the result shows at 3 months. The period when a review matters most is the period when a surgeon abroad cannot examine you. Our BBL recovery guide shows what normal healing looks like week by week.
Turkey BBL vs UK: How Do They Compare?
People often search for BBL in Turkey vs the UK, or Turkey vs Poland, Germany, Spain or the USA. The same comparison applies to any country, and these differences matter more than the price:
- Regulation. In England, hospitals performing cosmetic surgery are registered with and inspected by the Care Quality Commission (CQC), and reports are public. In Turkey, the Ministry of Health lists approved providers on its HealthTürkiye portal; the FCDO points to this but does not endorse any practitioner or facility[2].
- Surgeon registration. UK doctors must be registered with the General Medical Council (GMC), and you can check a surgeon’s registration online. Equivalent checks abroad are harder to read.
- Consultation and cooling off. The NHS recommends meeting your surgeon, two consultations and a cooling-off period of about two weeks[13]. Some packages abroad offer a video call with a coordinator and a first meeting with the surgeon on the day of surgery.
- Aftercare. UK surgery means your wound check, follow-up and any complication are handled by the team that operated.
- Recourse. UK patients have clear complaint routes and regulators. Overseas, you may need a lawyer in another country.
Our guide to the two-week cooling-off period explains why time between consultation and surgery matters.
How to Check a BBL Surgeon or Hospital Abroad
If you are looking for a safe BBL in Turkey or anywhere else, ask these questions in writing before paying a deposit:
- Which layer will the fat go into? The answer should be the subcutaneous layer only, above the gluteal muscle.
- Will real-time ultrasound be used throughout the fat transfer? Not whether the clinic has ultrasound, but whether it is used while fat is injected.
- Who is the operating surgeon, what is their plastic surgery training, how many BBLs do they perform a year, and can you meet them before the day?
- How much fat is planned for each side, and how long will the operation take?
- Is the hospital approved by the Ministry of Health, with overnight monitoring and intensive care on site?
- How long must you stay before flying, and what clot prevention is provided?
- What happens if a complication develops at home, who pays, and will you get your operation note in English?
Be wary of deposit deadlines, booking discounts, Instagram before-and-after photos with no surgeon named, and consent forms handed over on arrival. Our guide to choosing your Brazilian butt lift surgeon goes through each question in more depth.
What to Do If Something Goes Wrong After a BBL Abroad
Do not wait for a reply from the clinic abroad if you have any of these symptoms:
- Call 999 for breathlessness, chest pain or coughing up blood, which can be signs of a clot or fat embolism in the lungs.
- Contact NHS 111 or your GP the same day for calf pain or swelling, a fever, or a wound that is red, hot, leaking or opening.
The NHS will treat an emergency wherever you had surgery, but it does not usually correct cosmetic results such as asymmetry, dents or poor fat survival. Bring the operation note, the surgeon’s and hospital’s names, the fat volumes and your medicines. Missing details are common: one NHS breast unit found that the surgeon’s name had not been recorded for any of its patients returning from abroad[14].
If the result is uneven or has lost volume once healing has settled, a UK plastic surgeon can assess you privately. Centre for Surgery sees patients with complications or unsatisfactory results after a BBL abroad, starting with a face-to-face assessment of what can safely be done. Revision is possible but more complex than a first operation, as scar tissue makes each pass harder. Our guide to revision BBL explains what can be corrected.
If You Still Choose a BBL Abroad
Some people weigh all of this and still decide to travel. If you do:
- Tell your GP before you go, as the FCDO advises, and book a wound check for the week you return[2].
- Stay longer. Remain near the hospital until your surgeon has reviewed you, and avoid flying in the first week.
- Plan the journey. Book an aisle seat, wear compression stockings, walk regularly, drink water and avoid alcohol.
- Get specialist insurance. Standard travel insurance does not normally cover complications of surgery[13].
- Carry your records in English, including fat volumes, medicines and your discharge summary.
What Does a BBL in the UK Include?
At Centre for Surgery, every BBL places fat only in the subcutaneous layer above the gluteal muscle, never into or beneath it, under real-time ultrasound guidance with a blunt cannula and expansion vibration lipofilling (EVL). Fat is harvested with power-assisted liposuction or VASER, depending on your surgeon’s recommendation.
The operation is a day case under TIVA (total intravenous anaesthesia) and takes 3 to 4 hours. You are monitored for 1 to 3 hours before going home with a responsible adult, with your surgeon’s direct number for the first 24 hours. The nursing team stays in touch over the first two weeks, with a wound check at 7 to 14 days and a surgeon review at 6 weeks. Around 50 to 70% of the transferred fat usually survives, and the fat that remains is permanent from around 6 months.

A BMI of about 23 to 28 usually gives enough donor fat for a full BBL; if you are slimmer, a skinny BBL uses more donor areas for a modest, proportionate change.
Frequently Asked Questions
Is it safe to get a BBL in Turkey?
It can be, if an experienced plastic surgeon injects fat only above the gluteal muscle under real-time ultrasound, in a properly equipped hospital, and you stay long enough before flying. The risk is that these things are hard to verify from the UK, and aftercare usually ends when you leave. Most UK complications after cosmetic surgery abroad follow operations in Turkey.
How much is a BBL in Turkey in pounds?
Advertised packages are often less than half the price of a UK BBL and usually bundle a hotel, transfers and a translator. They rarely include revision, readmission or care for complications once you are home, which is where the real costs fall. Compare what each quote includes, not just the headline figure.
Why is BBL cheaper in Turkey?
Staff, property and running costs are lower, clinics operate at high volume, and medical tourism is actively promoted. Part of the saving also comes from what packages leave out: the named surgeon, ultrasound guidance, overnight observation, follow-up and complication care. Ask which of these are included before you pay.
How many people have died from a BBL in Turkey?
There is no official BBL-specific count. The UK Foreign Office says it is aware of 7 British nationals who died in Turkey in 2025 following medical procedures of all kinds. A coroner’s report into one British BBL death in Turkey found fat had been injected into muscle and raised concern that patients are not told of the risks.
How long after a BBL can you fly home?
The NHS advises avoiding flights for at least 5 to 7 days after liposuction, and a BBL is a bigger operation. You also need to avoid sitting directly on your buttocks for 2 weeks. Wear compression stockings, walk regularly on the flight, and follow your surgeon’s advice on clot prevention.
Will the NHS fix a BBL done abroad?
The NHS treats emergencies such as infection, wound breakdown, blood clots and sepsis, whatever caused them. It does not correct cosmetic outcomes such as asymmetry, dents or lost volume. Those need a private assessment, and revision after a BBL elsewhere is more complex than a first operation.
Which country is best for a BBL?
No country is safest by default. The safest BBL is one where fat is placed only above the gluteal muscle under real-time ultrasound, by a surgeon you have met, with aftercare you can reach easily. For UK residents, surgery close to home makes follow-up and complication care far simpler.
Who is the best BBL surgeon in Turkey?
Look beyond Instagram results. Confirm the surgeon’s plastic surgery training and registration, meet them before the day of surgery, and ask in writing about the injection plane, ultrasound use, fat volumes and complication care. Check the hospital on the Ministry of Health’s HealthTürkiye portal, and avoid deposit deadlines.
Is a skinny BBL in Turkey safe?
A skinny BBL carries the same embolism risk as any BBL, so the same rules apply: fat above the muscle only, placed under ultrasound. Slim patients need fat from more donor areas, which lengthens surgery, so flying home soon afterwards needs particular care and a longer stay before travel.
